If you need other accommodations to support your participation, let us know on the registration page by Friday, September 26, 2025 by midnight at your local time zone. We will try, but cannot guarantee accommodation requests made after that date.
We invite staff from health centers (HCs) including CHWs and frontline staff, primary care associations (PCAs), and collaborative partners within local/state/territory community programs, including anti-domestic violence and human trafficking programs, to apply.
We encourage all applicants to identify one staff member as the main applicant and submit one application per site. You may add multiple colleagues to the registration.
Please note that we will schedule one, 15–20-minute conversation with some accepted applicants before the CoP begins. This is an opportunity to provide input on the CoP and share any related efforts to date.
